Evercare Health and Home Connection™: Care Manager coordinates medical and non-medical services, including personal care, respite services, preventive dental care, coverage for prescription drugs excluded by Medicare Part D, over-the-counter medications, personal care items and durable medical equipment. Available in 20 counties including Broward, Miami-Dade and Palm Beach.
Evercare At Home™: Care Manager coordinates medical and non-medical services, including preventive care, adult day care; and includes prescription coverage for people not eligible for Medicare Part D benefits. Available in Broward and Miami-Dade Counties.
